A team retrospective board: three columns (went well, didn't go well, try next, or your own), anonymous cards anyone can add, dot voting with a per-person budget, each column sorted by dots, the most-voted cards pulled to the top, questions for the facilitator and the actions you agreed. Cards and dots are stored as rows you can read back.

The retro: a title and team line, a short intro, two to five columns (each with an id, a title, a prompt and a tone), how many dots each person gets, any seed cards gathered before the meeting, questions for the facilitator, and the actions agreed afterwards. Column ids must equal the declaration's column choices. Cards and dots people add are rows, not part of this document.
